Please fill out the form below and we will call you when your copy is ready.
You can then pick up your copy at the pick-up box located outside of our
office.
You can leave your check in the box when you pick up your DVD or tape. Please
do not leave cash.
If you would like us to mail your copy to you the fee is $15.00.
